Deductibles – $1250, $2500, $3000, $3500, or $5000 for individuals.  $2500, $5000, $6000, $7000, or $10,000 for families.  Benefits paid 100% after deductible (except preventative care, see brochure for details).

Office Visits – 100% after deductible, unlimited visits.

Lifetime Maximum
– Unlimited

Preventative Care for Children
– 100% before deductible, no waiting periods, co-payments or out of pocket expenses required for covered services.

Preventative Care for Adults and Children age 7 and up
– 100% before deductible, no waiting periods, co-payments or out of pocket expenses required for covered services.

Prescription Drugs
– Brand name and generic both covered, unlimited benefit (this is a VERY important benefit!)Paid 100% after deductible.

Professional & Diagnostic Services
– 100% after deductible

Inpatient Services
– 100% after deductible. Overnight hospital or facility stays.

Outpatient Services
– 100% after deductible.  Services without overnight stays.

Emergency Room Services
– 100% after deductible

Maternity – Not covered.
